c.) Completed PACW Test. CLICK HERE
If you are a minor (under the age of 18), OR you are bringing a minor that is not your own child you also need:
a.) Minor Consent to Treatment form http://www.ivrmc-rsmc.com/img.php/thirdpartyminor.pdf? RevisionID=4432&Filename=thirdpartyminor.pdf
b) A release letter from the/your parents/guardian that gives temporary guardianship of the minor to an adult that is going to be at the event. Please include dates and contact numbers.
3.) Black comfortable shoes. Black work boots, or even solid black tennis shoes will do.
4.) Clothes to sleep in.
5.) Sleeping bag and maybe an extra blanket. (Check the weather report)
6.) Extra Socks
7.) Personal toiletries such as deodorant, toothbrush and paste, and any medications.

9.) A few dollars cash in case you want to buy something.
10.) A great attitude and willingness to have some FUN!
THINGS YOU DO NOT NEED YOUR FIRST TIME
1.) Do not worry about food as it will be provided for you.
2.) You do not need plates, forks etc as we have them for you too!
3.) You do not need worry about having the right uniform as we have loaner gear.
4.) A negative attitude.
